The HiRes 90K Operative Report is a two-page document to be completed for each implant following surgery. The report should be sent to Advanced Bionics, and a copy should be kept for your clinic’s files.

Implantable Cochlear Stimulator (ICS) Device Registration Form PDF (57.5 KB)>>
Following surgery, newly implanted devices should be registered using the ICS Device Registration Form. Advanced Bionics accepts either the original form included in the implant kit or a copy of the device registration form faxed to Advanced Bionics Customer Care: (661) 362-7795.
